You should file a complaint with the state Insurance Commission (Department of Insurance or DOI) if you've tried and failed to resolve issues like claim denials, delays, or unfair treatment directly with your insurance company, especially after providing documentation and cooperating.

Your state's DOI can investigate disputes over claims, coverage, premiums, or agent conduct.
